You will be responsible for identifying and resolving barriers that hinder effective care, facilitating the development of trusting relationships with members to enhance the quality and continuity of care. Collaboration with the Service Coordinator, Transition Coordinator, and other care team staff will be essential to address the identified needs of each member.
