We’re seeking a compassionate and clinically strong Registered Nurse (RN) Case Manager to support members through complex health journeys. In this role, you’ll use your clinical expertise and critical thinking to guide care, remove barriers, and improve overall health outcomes—while working in a flexible, structured environment. What You’ll Do Conduct comprehensive assessments using clinical tools, data, and member interaction Develop proactive care plans that address medical, social, and functional needs Identify and reduce health risks while coordinating care across programs and services Review claims and clinical history to support effective case management Engage members via phone to understand needs and drive participation in care plans Partner with interdisciplinary teams and participate in case conferences Ensure compliance with regulatory and organizational guidelines What You Bring - REQUIRED Active, unrestricted RN license in your state of residence Willingness to obtain additional state licenses (fully funded) 3+ years acute care experience (med-surg, ICU, or post-surgical) Strong clinical judgment and communication skills Comfort with Microsoft Office tools and telephonic care management Proficiency with computer systems and technology (MS Office Suite) Associates Degree in Nursing

Preferred Qualifications

Case management or home health experience Compact RN license Certified Case Manager (CCM) credential BSN degree Schedule & Flexibility Monday–Friday, typically 8:00 AM – 4:30 PM (local time) Flexible scheduling between 8:00 AM – 6:00 PM (8 hour shifts) Occasional rotating late shift (until 7:00 PM) Minimal travel (Apply To This Job
