Clinical Social Worker (LMSW or LCSW) – Master’s Degree Required

Responsibilities
- Provide in-home visits supporting patients and families.
- Deliver counseling, care planning, and crisis intervention.
- Connect patients to community resources and support systems.
- Collaborate with an interdisciplinary care team.
Requirements
- Master’s Degree in Social Work (MSW) required.
- Active LGSW or LISW license in Minnesota.
- At least 1 year of clinical Social Work experience.
- A passion for independent, relationship-based care.
