Provide medication therapy management services to UnitedHealth Group members, utilizing evidence-based medicine and a patient-centered approach. Conduct comprehensive medication reviews and identify opportunities for optimizing medication therapies. Collaborate with healthcare providers to develop and implement individualized treatment plans. Monitor and evaluate patient outcomes and make recommendations for medication adjustments as needed. Stay current on industry developments, regulations, and guidelines related to medication therapy and healthcare. Utilize advanced clinical knowledge and skills to identify and resolve potential medication-related issues. Participate in quality improvement initiatives and provide input on strategies to enhance patient outcomes. Maintain accurate and timely documentation of all clinical activities and interventions. Serve as a resource and mentor for other team members, including pharmacy technicians and other pharmacists. Contribute to the development and implementation of new programs and initiatives within the Rx Clinical Insights team. Communicate effectively with patients, healthcare providers, and other stakeholders to promote medication adherence and improve health outcomes. Adhere to all company policies, procedures, and regulatory requirements. Continuously strive to improve processes and workflows to increase efficiency and effectiveness. Participate in ongoing training and development to enhance clinical knowledge and skills. Represent UnitedHealth Group in a professional and positive manner at all times.
