Responsibilities

  • Oversee remote patient monitoring services for individuals with chronic illnesses.
  • Collaborate with Family Medicine Care Coordinators and interdisciplinary teams.
  • Develop and implement personalized health care plans for patients.
  • Facilitate effective communication between patients and health care providers.
  • Monitor patient progress and adjust care plans as necessary.
  • Provide education and resources to patients and their families.
  • Document and maintain accurate patient records and data.
  • Ensure compliance with healthcare regulations and standards.

Requirements

  • Bachelor’s degree in Health Education or related field, or California LVN license.
  • Experience in a healthcare setting preferred.
  • Strong written and verbal communication skills.
  • Proficient in Microsoft Office and electronic health record systems.
  • Ability to work independently and as part of a team.
  • Excellent organizational skills and attention to detail.
  • Fluency in English and Spanish is a plus.
  • Ability to manage confidential information with discretion.
